The authors are to be congratulated in their application of the BCT taxonomy to the systematic review process in the context of home-based cardiac rehabilitation. Interestingly the paper gave a list of groups of the behaviour change techniques that were not used in the studies, including self-belief. On the contrary we would argue that self belief is indeed core to the Heart Manual intervention particularly BCT 40, verbal persuasion about capability which is described in the taxonomy as ‘tell the person they can successfully perform the wanted behaviour arguing against self doubts and asserting that they can and will succeed’ the example given ‘ tell the person that they can successfully increase their physical activity despite their recent heart attack’. The Heart Manual facilitators are trained to use this technique whilst facilitating the manual with cardiac patients.The fact that we can point out this discrepancy emphasises the usefulness of reference to an agreed taxonomy in this context. It also highlights the difficulty in accurately describing a complex intervention with all its ‘active ingredients’ within the limited space of a journal article without such.
