It may be a simple strategy to invite patients to a weight management programme by letter, but we know that this generally is not effective in patients who are disadvantaged.The text of the article states that studies demonstrating an opportunistic message were most effective. This is consistent with what we know about deprivation, in that patients do not respond to the written word, often are intimidated or alienated, or may not fully understand it. The bias to uptake may be partly clinician based as is suggested but there may also there may be a patient element. I fully agree with the online conclusion that paying more attention to inviting deprived patients is a more effective policy, but an opportunistic face to face problem-solving approach may be the better one, also avoiding stigmatising patients by letters addressed to overweight patients.Competing Interests: None declared.
