Yes, I totally agree that my life as a GP is affected by QOF. Let us though differentiate between GP salary and clinical care. As a non principle my income is not currently affected by QOF.GP payment has always been a very muddled area and is made up of pots of different pots. Many years ago it included contraceptive payments and night visit payments. The government moved the way money moved and called some of it QOF.Yes, the better care that was provided the more the practices earned. Yes QOF has some odd requirements that make little sense or may be harmful (ie HBA targets) and it is one of the GP roles to make sure they used patient centred care and best evidence for that care not QOF targets.But not let us ignore the huge reduction in premature cardiovascular disease, let us not ignore that practice can now be judged on how good the measurable outcomes are. In any given consultation QOF is not the prime aim or agenda unless it is for a 'QOF review'.Sadly in the UK people don't seem to want to remember their BP, their PSA, their cholesterol etc. or often even their weight or BMI.Whether the state via the NHS should tackle this issue may depend on ones own views.
