Exploring the impact of employment and deployment models of new roles on staff embeddedness and integration with GP practices

Abstract

Background The Additional Roles Reimbursement Scheme (ARRS) has introduced an extended number of new roles working in general practice. There are currently 18 types of ARRS staff, including clinical pharmacists, paramedics, first-contact physiotherapists, and social prescribers. From 2024/2025, the scheme will fund recently qualified GPs. However, the different ways ARRS staff are employed and deployed impact their embeddedness and integration with GP practices.

Aim This study explores the impact of ARRS staff employment and deployment models on staff embeddedness and integration with GP practices.

Method A qualitative case study was conducted across four Primary Care Networks (PCNs) in England using a realist evaluation framework. Data collection took place between September 2022 and November 2023. Semi-structured interviews were conducted with PCN Clinical Directors, GPs, practice managers and ARRS staff (n = 42). Transcripts were analysed using a realist evaluation framework to identify the context-mechanism-outcome configuration.

Results Our study found variations in how ARRS staff were employed and deployed across the four PCNs. ARRS staff are either directly employed by GP practices or subcontracted from hospitals or third-sector organisations. ARRS staff rotated across different numbers of GP practices. These differences in employment and deployment models had a notable impact on their relationships with existing GP practice staff.

Conclusion This study offers valuable insights into how different employment and deployment models influence the experiences of ARRS staff. These findings have implications for ARRS staff retention, particularly as it extends to include recently qualified GPs.
