This paper addresses the issues of validity, describing the technique as it has evolved over the past three years, and paying particular attention to ways in which the consultation was made to feel 'real' for candidates. As well as testing clinical issues during consultations, simulated surgery incorporates the views of the patient into the assessment process. Feedback from registrars has been positive and the method presents an alternative to video-recording the consultation for the purpose of assessment.
