    The NRLS was launched in 2003 and receives approximately 100,000 reports a month, the majority written by healthcare professionals. It receives over 65,000 reports per annum involving children [13]. Reporting patient safety incidents that resulted in severe harm or death of a patient became mandatory in June 2010; however, before this all reporting was voluntary, and reporting remains voluntary for incidents resulting in no, low, or moderate harm [14,15].
